In einigen Fällen (z.B.Verschluss der Samenleiter) finden sich im Ejakulat überhaupt keine Samen-zellen. Die Samenzellen können aber dennoch direkt aus dem Hoden (TESA - testikuläre Spermienaspiration), bzw. dem Nebenhoden (MESA – mikorchirurgische epididymale Spermienaspiration) entnommen werden. An unserer Klinik werden diese beiden kleinen operativen Eingriffe zur Entnahme einer Gewebeprobe, die Samenzellen enthält, unter Voll-narkose durchgeführt. Es reichen schon einige wenige Spermien aus, um mit Hilfe des ICSI-Ver-fahrens gute Ergebnisse bei der Befruchtung zu erzielen.
Hat die Befruchtung stattgefunden, gibt es keine signifikanten Unterschiede zwischen dem kombinierten ICSI/TESA/MESA-Verfahren und der herkömmlichen IVF hinsichtlich der Qualität des Embryos und der Schwangerschaftsrate.
Bei schwerer nicht-obstruktiver Oligozoospermie sowie bei obstruktiver und nicht-obstruktiver Azoospermie hat die Fruchtbarkeitsstörung möglicherweise genetische Ursachen. Es besteht dann das Risiko, dass sie an Söhne weitergegeben wird oder dass es zu anderen genetisch-be-dingten Erkrankungen bei den Kindern kommen könnte. Liegt ein solcher Fall vor, erhält das Paar an unserer Klinik eine medizinisch-genetische Beratung und Untersuchung (Karyo-typisierung, AZF-Mikrodeletion, Mukovizidose). Nur durch solch einen komplexen Ansatz kann die Geburt eines gesunden Kindes sichergestellt werden.
