{"id":7739,"date":"2020-03-09T22:05:05","date_gmt":"2020-03-09T22:05:05","guid":{"rendered":"https:\/\/olgafertilityclinic.com\/your-ivf-process-one-step-at-a-time\/"},"modified":"2024-04-09T08:34:58","modified_gmt":"2024-04-09T08:34:58","slug":"ivf","status":"publish","type":"page","link":"https:\/\/olgafertilityclinic.com\/de\/ivf\/","title":{"rendered":"Wie kann man IVF-Misserfolge verhindern?"},"content":{"rendered":"<ul id=\"tocList\" class=\"toclist\"><\/ul>\n<p style=\"font-weight: 400;\">Um eine Antwort auf die Frage zu finden, wie wir die Zahl der erfolgreichen IVF noch steigern k\u00f6nnen, m\u00fcssen wir zun\u00e4chst verstehen, aus welchen Gr\u00fcnden IVF scheitern.<\/p>\n<p style=\"font-weight: 400;\">Unsere Patientinnen haben oft mehr als einen Grund, warum sie noch nicht schwanger geworden sind oder warum sich die Schwangerschaften nicht bis zur vollen Dauer entwickelt haben. Andernfalls h\u00e4tten die ersten IVF-Versuche in&nbsp;ihren \u00f6rtlichen Kliniken bereits zu einem Ergebnis gef\u00fchrt, und wir h\u00e4tten uns nie getroffen.<\/p>\n<p style=\"font-weight: 400;\">Um jeden Grund f\u00fcr das Scheitern einer IVF zu beseitigen oder zu korrigieren, halten wir uns an das Prinzip \u201eF\u00fcr jede Herausforderung gibt es eine L\u00f6sung&#8221;.<\/p>\n<p><span style=\"font-weight: 400;\">Dieser Ansatz ist nicht neu: Wenn man ein gro\u00dfes Projekt hat, neigt man dazu, es in&nbsp;mehrere kleinere Projekte mit kleineren Zielen aufzuteilen, die zu einem gro\u00dfen Ziel f\u00fchren. Die kleineren Ziele sind hier: die Eizellen, die Blastozysten, die chromosomal normalen Blastozysten, die vorbereitete Geb\u00e4rmutter, das fertige Implantationsfenster und schlie\u00dflich eine chromosomal normale Blastozyste in&nbsp;die fertige Geb\u00e4rmutterschleimhaut zum richtigen Zeitpunkt einzusetzen.<\/span><\/p>\n<h3>Das einzige gro\u00dfe Ziel ist ein Baby<\/h3>\n<p style=\"font-weight: 400;\">Die Gr\u00fcnde f\u00fcr das Scheitern der IVF und den Schwangerschaftsverlust lassen sich in&nbsp;zwei Gruppen einteilen: <strong>embryonale<\/strong> und <strong>m\u00fctterliche Ursachen<\/strong>.<\/p>\n<p style=\"font-weight: 400;\"><strong>Embryonale Ursachen<\/strong> f\u00fcr IVF-Versagen und Schwangerschaftsverlust bedeuten, dass es im Inneren des Embryos einen Defekt gibt, der ihn nicht lebens- und entwicklungsf\u00e4hig macht, selbst unter den besten m\u00fctterlichen Bedingungen.<\/p>\n<p style=\"font-weight: 400;\"><strong>M\u00fctterliche Ursachen<\/strong> f\u00fcr das Scheitern der IVF und den Schwangerschaftsverlust umfassen Ursachen innerhalb des weiblichen Organismus und k\u00f6nnen durch hormonelle, immunologische oder uterine Faktoren erkl\u00e4rt werden.<\/p>\n<h2 class=\"notclear\">\u00dcberwindung der embryonalen Ursachen f\u00fcr Schwangerschaftsverlust und IVF-Versagen<\/h2>\n<p>Embryonale Gr\u00fcnde f\u00fcr den Schwangerschaftsverlust und das Scheitern der IVF sind am st\u00e4rksten und zu mindestens 75&nbsp;% f\u00fcr das Ergebnis der Fruchtbarkeitsbehandlung und die Schwangerschaft verantwortlich.<\/p>\n<p><strong>Abbildung 1. Lebensjahr haben oft einen abnormalen Chromosomensatz [Fragouli et al., 2012], und das ist, so&nbsp;seltsam es klingen mag, normal.<\/li>\n<li>Ein abnormaler Chromosomensatz im Embryo f\u00fchrt fast immer zu einem Schwangerschaftsverlust in&nbsp;der Pr\u00e4implantationsphase (Implantationsversagen) oder in&nbsp;der Postimplantationsphase (Fehlgeburt) [Scott at&nbsp;al., 2012].<\/li>\n<li>Ein abnormaler Chromosomensatz ist fast immer m\u00fctterlichen Ursprungs, d. h. der Embryo erh\u00e4lt ihn von der Eizelle.<\/li>\n<li>Je \u00e4lter die Eizellen sind, desto wahrscheinlicher ist es, dass der Embryo Chromosomenanomalien aufweist, was wiederum normal ist.<\/li>\n<li>Der Einfluss von Chromosomenfehlern auf die Morphologie des Embryos (wie der Embryo unter dem Mikroskop aussieht) ist am 5.-6. Entwicklungstag unbedeutend. Das bedeutet, dass der Embryo zwar normal aussieht, aber einen abnormalen Chromosomensatz hat [Alfarawati et al., 2011]<\/li>\n<\/ul>\n<h4>Welcher Anteil der Embryonen im Blastozystenstadium wird voraussichtlich einen normalen Chromosomensatz haben?<\/h4>\n<p><strong>Diagramm 1. Es gibt diese M\u00f6glichkeit, die PGT-A genannt wird.<\/p>\n<\/div>\n<h4>\nWas ist PGT-A?<\/h4>\n<p style=\"font-weight: 400;\">PGT-A steht f\u00fcr <strong>P<\/strong>reimplantation <strong>G<\/strong>enetic <strong>T<\/strong>esting for<strong> A<\/strong>neuploidies (Aneuploidie \u2013 ein abnormer Chromosomensatz). Bei dieser genetischen Untersuchung von Embryonen werden die normalen Embryonen unter den verf\u00fcgbaren Embryonen ausgew\u00e4hlt. Durch den Ausschluss abnormaler Embryonen wird die Lebendgeburtenrate pro \u00fcbertragenem Embryo erh\u00f6ht. Dies geschieht, indem die Tests die Embryonen mit abnormalem Chromosomensatz identifizieren, welche zu Einnistungsfehlern und Schwangerschaftsverlusten f\u00fchren w\u00fcrden, und diese nicht in&nbsp;die Geb\u00e4rmutterh\u00f6hle \u00fcbertragen werden.<\/p>\n<h4>Was ist der Zweck von PGT-A?<\/h4>\n<p style=\"font-weight: 400;\">PGT-A macht die Embryonen nicht besser. Fertility bei 51%. Das bedeutet, dass in&nbsp;unserer Klinik 51% der Transfers eines einzelnen Embryos mit normalem Chromosomensatz zur Geburt eines Kindes f\u00fchren (Diagramm 4).<\/p>\n<p class=\"pic\">\nDiagramm 4. In&nbsp;vielen F\u00e4llen (wiederholte Schwangerschaftsverluste und fehlgeschlagene Einnistungen) erh\u00f6ht PGT-A auch den IVF-Erfolg bei Frauen unter 35 Jahren.<\/p>\n<p><\/span><\/div>\n<p class=\"getcons\"><a class=\"button contactus ui large\" href=\"#footerform\">Kontaktieren Sie uns<\/a><\/p>\n<h2>II. M\u00fctterliche Ursachen f\u00fcr Schwangerschaftsverlust und IVF-Versagen<\/h2>\n<p style=\"font-weight: 400;\">Die m\u00fctterlichen Ursachen f\u00fcr den Schwangerschaftsverlust und das Scheitern der IVF k\u00f6nnen hormoneller, entz\u00fcndlicher oder immunologischer Natur sein. Die Ursachen k\u00f6nnen mit dem Zustand der Organe des Fortpflanzungssystems oder anderer Organe und Systeme des K\u00f6rpers zusammenh\u00e4ngen. Oft gibt es bei unseren Patientinnen mehrere Ursachen, warum eine anhaltende Schwangerschaft und eine Lebendgeburt noch nicht erreicht werden konnten.<\/p>\n<h3>II.1. Verpassen des Einnistungsfensters \u2013 der Hauptgrund f\u00fcr das Scheitern der Einnistung in&nbsp;frischen IVF-Zyklen<\/h3>\n<p style=\"font-weight: 400;\">Der Vorgang, bei dem sich der Embryo in&nbsp;der Geb\u00e4rmutter festsetzt, wird als Implantation bezeichnet. Die Einnistung beginnt mit der Erkennung bestimmter Rezeptoren auf der Oberfl\u00e4che der Geb\u00e4rmutterschleimhaut durch Rezeptoren auf der Oberfl\u00e4che des geschl\u00fcpften Embryos.<\/p>\n<p style=\"font-weight: 400;\">Das Einnistungsfenster ist der Zeitraum, in&nbsp;dem diese Rezeptoren in&nbsp;der Geb\u00e4rmutterschleimhaut vorhanden sind, in&nbsp;der Regel 5-7 Tage nach dem Beginn der Progesteronproduktion durch das Eibl\u00e4schen.<\/p>\n<p style=\"font-weight: 400;\">Normalerweise beginnt die Progesteronproduktion in&nbsp;einem nat\u00fcrlichen Zyklus mit dem Tag des Eisprungs. Daher sind die Rezeptoren auf den Zellen der Geb\u00e4rmutterschleimhaut in&nbsp;der Regel 5-7 Tage nach dem Eisprung bereit f\u00fcr die Begegnung mit dem Embryo. Gleichzeitig ist der Embryo, nachdem er den Prozess des Schl\u00fcpfens aus der H\u00fclle vollzogen hat, 5-7 Tage nach dem Eisprung bereit f\u00fcr die Einnistung.<\/p>\n<p style=\"font-weight: 400;\">\u00a0<\/p>\n<h4>Die Natur hat diese beiden Prozesse der embryonalen und endometrialen Bereitschaft zur Einnistung sehr geschickt synchronisiert:<\/h4>\n<ul>\n<li>Der Embryo ist 5-7 Tage nach dem Eisprung bereit f\u00fcr die Einnistung (Bild 3).<\/li>\n<li>Die Geb\u00e4rmutterschleimhaut ist 5-7 Tage nach dem Eisprung bereit f\u00fcr die Einnistung (Bild 4).<\/li>\n<\/ul>\n<h4>Abbildung 3. Dies geschieht aufgrund der supraphysiologischen Hormonkonzentration im K\u00f6rper. In&nbsp;vielen stimulierten Zyklen beginnt die Progesteronproduktion zu fr\u00fch \u2013 bereits mehrere Tage vor der Eizellentnahme (was dem Eisprung entspricht).<\/p>\n<p style=\"font-weight: 400;\">Wenn die Progesteronproduktion fr\u00fcher als am Tag der Embryonenbildung (Eizellentnahme oder Tag des Eisprungs) einsetzt, sind die Rezeptoren in&nbsp;der Geb\u00e4rmutterschleimhaut, die den Embryo h\u00e4tten erkennen k\u00f6nnen, fr\u00fcher vorhanden. Wenn der Embryo das Stadium des Schl\u00fcpfens der Blastozyste erreicht, sind die Rezeptoren in&nbsp;der Geb\u00e4rmutterschleimhaut nicht mehr vorhanden und der Dialog zwischen Embryo und Geb\u00e4rmutterschleimhaut ist nicht mehr m\u00f6glich. Wiederherstellung des Dialogs zwischen dem Embryo und der Geb\u00e4rmutter, indem Sie in&nbsp;Ihrem IVF-Prozess einen Schritt nach dem anderen machen<\/h3>\n<p style=\"font-weight: 400;\">Der fehlende Dialog zwischen dem Embryo und der Geb\u00e4rmutterschleimhaut kann vermieden werden, indem eine Aufgabe nach der anderen erledigt wird.<\/p>\n<p style=\"font-weight: 400;\">Der erste Schritt besteht darin, durch eine individuell abgestimmte Stimulation kompetente Eizellen und lebensf\u00e4hige Embryonen zu erzeugen. Bei dieser Stimulation konzentrieren wir uns ausschlie\u00dflich auf die Follikel, Eizellen und Embryonen. Es ist noch nicht m\u00f6glich, sich auch um die Geb\u00e4rmutterschleimhaut zu k\u00fcmmern.<\/p>\n<p style=\"font-weight: 400;\">Wenn die Blastozysten fertig sind, halten wir die Zeit f\u00fcr sie an, indem wir sie schonend einfrieren. Wenn wir die Zeit f\u00fcr die Embryonen angehalten haben, k\u00f6nnen wir uns auf die optimale Vorbereitung der Geb\u00e4rmutter f\u00fcr den Embryotransfer konzentrieren.<\/p>\n<p style=\"font-weight: 400;\">F\u00fcr den Embryotransfer w\u00e4hlen wir einen separaten Zyklus, in&nbsp;dem wir uns auf die Geb\u00e4rmutterschleimhaut konzentrieren, mit dem Ziel, zum optimalen Zeitpunkt einen Embryo in&nbsp;die optimal vorbereitete Geb\u00e4rmutterschleimhaut zu \u00fcbertragen. Normalerweise ist nach dem stimulierten Zyklus ein Menstruationszyklus erforderlich, damit die Eierst\u00f6cke keine Restmengen an Progesteron mehr produzieren. Daher empfehlen wir eine Pause von einem Zyklus zwischen der Eizellentnahme und dem Embryotransfer.<\/p>\n<p class=\"pic\">\nDiagramm 5.
