Dieser Test zeigt die Anzahl und Struktur der Chromosomen (die genetisches Material tragen) in&nbsp;jedem getesteten Embryo.<\/p>\n<p>Normalerweise haben Embryonen 23 Chromosomenpaare (euploide Embryonen). Aber fast die H\u00e4lfte der Embryonen wird eine abnorme Chromosomenzahl aufweisen (aneuploide Embryonen). Wenn ein zus\u00e4tzliches oder fehlendes Chromosom im Embryo gefunden wird, ist es weniger wahrscheinlich, dass es zur Einnistung kommt. H\u00e4ufig kommt es zu einer Fehlgeburt oder in&nbsp;einigen F\u00e4llen k\u00f6nnen sie sich noch zur vollen Reife entwickeln, aber es wird ein Baby mit einer genetischen Erkrankung geboren (z.B. Down-Syndrom &#8211; ein zus\u00e4tzliches Chromosom 21).<\/p>\n<p>Beim PGT-A wird die Anzahl der Chromosomen in&nbsp;der von Embryonen entnommenen DNA getestet. Embryonen mit einer abnormen Anzahl von Chromosomen f\u00fchren nicht zu einer gesunden Lebendgeburt, daher sind sie von der Verwendung ausgeschlossen. Embryonen mit einer normalen Anzahl von Chromosomen w\u00fcrden f\u00fcr den Embryotransfer verwendet werden.<\/p>\n<table>\n<tbody>\n<tr>\n<td style=\"text-align: center;\"><strong>PGT-A macht die Embryonen nicht besser, der Test zeigt, welche von den verf\u00fcgbaren Embryonen eine normale Anzahl von Chromosomen haben<\/strong><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>Das Chromosomen-Screening hilft bei der Auswahl des besten Embryos f\u00fcr den Transfer &#8211; Ihre Chancen, schwanger zu werden, sind also h\u00f6her und die Fehlgeburtenraten niedriger, nachdem nicht nur ein gut aussehender, sondern auch ein euploider Embryo \u00fcbertragen wurde. Die Embryonenbiopsie ist trotz ihrer Sicherheit ein invasives Verfahren. Es handelt sich um einen weiteren zus\u00e4tzlichen Schritt bei der IVF, so&nbsp;dass vor der Durchf\u00fchrung eine solide Indikation \u00fcber die Vorteile gestellt werden muss.<\/p>\n<p>In&nbsp;mehreren Patientengruppen gibt es unbestreitbare Vorteile f\u00fcr die Anwendung von PGT-A.<\/p>\n<h4>Fortgeschrittenes m\u00fctterliches Alter<\/h4>\n<p>Es ist gut dokumentiert, dass der chromosomal anomale Status eines Embryos haupts\u00e4chlich (zu mehr als 90%) durch die Eizelle mitbestimmt wird. Das m\u00fctterliche Alter ist entscheidend f\u00fcr die genetische Konstitution der Eizelle und damit des Embryos, da der Embryo von der Eizelle abstammt. Anteil von Blastozysten mit einer normalen Anzahl von Chromosomen bei Frauen verschiedener Altersgruppen. <\/strong>[Munne et al., 2015]<\/p>\n<p>Verschiedenen umfangreichen Studien zufolge haben Frauen im Alter von 35 Jahren und \u00e4lter im Vergleich zur Kontrollgruppe einen signifikanten Anstieg der Lebendgeburtenrate pro Embryo, der nach einer PGT-A \u00fcbertragen wurde. Auf der anderen Seite profitieren j\u00fcngere Frauen unter 35 Jahren nicht viel von der PGT-A. Pregnancy outcomes<\/strong><\/p>\n<p><em>STAR \u2014 20-Wochen-Schwangerschaftsrate pro Transfer, wobei zu beachten ist, dass alle 20-Wochen-Schwangerschaften zu einer Lebendgeburt f\u00fchrten<br \/>\n<\/em><em>SART \u2014 Lebendgeburtenrate pro eSET<\/em><\/p>\n<p><strong>PGT-A steigert bei Eizellspende oder Embryoadoptionszyklen die Erfolgsraten nicht, da Eizellen von j\u00fcngeren Frauen verwendet werden. In&nbsp;Eizellspende- und Embryonenadoptionszyklen erh\u00f6ht PGT-A nur die Kosten des Erfolges.<\/strong><\/p>\n<h4>Wiederkehrender fr\u00fchzeitiger Schwangerschaftsverlust<\/h4>\n<p>Mehrfache, erfolglose IVF-Versuche und Fehlgeburten sind f\u00fcr die Patienten sehr \u00e4rgerlich und frustrierend. Wenn die Ursache der fr\u00fcheren Misserfolge aneuploide Embryonen waren, dann profitieren auch diese Patienten von einer PGT-A.<\/p>\n<h4>Chromosomenst\u00f6rungen im Karyotyp eines der Patienten<\/h4>\n<p>Wenn Sie oder Ihr Partner ausgewogene chromosomale Ver\u00e4nderungen aufweisen, sind Ihre Embryonen einem gr\u00f6\u00dferen Risiko der Aneuploidie ausgesetzt. Bei der Biopsie entnimmt ein Embryologe 3-7 Zellen aus dem \u00e4usseren Teil der Blastozyste (Trophektoderm, TE). Die innere Zellmasse (ICM) der Blastozyste bleibt intakt, so&nbsp;dass dem zuk\u00fcnftigen Baby kein Schaden zugef\u00fcgt wird. Die Biopsien von jeder Blastozyste werden markiert und zur Analyse an ein Genetiklabor geschickt. Dann werden die Blastozysten eingefroren und einzeln gelagert.<\/div>\n<\/li>\n<li>\n<div><strong>Embryo Biopsy + Freezing<\/strong><br \/>\nBlastocysts with good morphology are biopsied on&nbsp;Days 5-6. During the biopsy an embryologist takes 3-7 cells from the outer part of the blastocyst (trophectoderm, TE). The inner cell mass (ICM) of the blastocyst remains intact, so&nbsp;there is no harm to the future baby. Biopsies from each blastocyst are marked and&nbsp;sent to the genetics lab for&nbsp;analysis. ICM \u2014 Innere Zellmasse (zuk\u00fcnftiges Baby), TE \u2014 Trophectoderm (zuk\u00fcnftige Placenta) <\/strong><\/p>\n<div>\u00a0<\/div>\n<\/div>\n<\/li>\n<li>\n<div><strong>Ergebnisse der genetischen Tests<\/strong><br \/>\nEs gibt mehrere Methoden f\u00fcr PGT-A, die modernste und zuverl\u00e4ssigste ist das Next Generation Sequencing (NGS). Es dauert etwa zwei Wochen, bis die Ergebnisse aus dem genetischen Labor f\u00fcr jeden Embryo vorliegen.<\/div>\n<\/li>\n<li>\n<div><strong>Transfer eingefrorener Embryo<\/strong><br \/>\nWenn die PGT-A Ergebnisse vorliegen und nach diesem Ergebnis mindestens ein euploider Embryo verf\u00fcgbar ist, beginnt die Vorbereitung der Geb\u00e4rmutterschleimhaut f\u00fcr den Embryotransfer. Am Tag des Transfers taut ein Embryologe den ausgew\u00e4hlten Embryo auf und bereitet ihn f\u00fcr den Transfer vor.<\/div>\n<\/li>\n<\/ol>\n<p class=\"getcons\">Haben Sie Fragen?<\/p>\n<p class=\"getcons\"><a class=\"button contactus ui large\" href=\"#footerform\">Erhalten Sie Eine Kostenfreie Konsultation!<\/a><\/p>\n<h2>Wie sind die Ergebnisse von PGT-A zu interpretieren?<\/h2>\n<p>Die Sensitivit\u00e4t (keine falsch-negativen Ergebnisse) und Spezifit\u00e4t (keine falsch-positiven Ergebnisse) der NGS-Methode des PGT-A liegen bei 95-98%, wie von verschiedenen Autoren best\u00e4tigt wurde. Das bedeutet, dass Sie auf die Ergebnisse vertrauen k\u00f6nnen, aber auch, dass keine diagnostische Methode eine 100%ige Genauigkeit bieten kann. Embryonen sind hochdynamische Gebilde, embryonale Zellen teilen sich sehr aktiv, so&nbsp;dass bei diesen Teilungen genetische Fehler auftreten k\u00f6nnen. Dies kann zu einem Mosaizismus des Embryos f\u00fchren &#8211; verschiedene Zellen im Embryo k\u00f6nnen einen unterschiedlichen Chromosomensatz haben. Aber bei der Biopsie werden nur 3-7 Zellen der Blastozyste entnommen, so&nbsp;dass die Frage lautet: &#8220;Sind diese Zellen repr\u00e4sentativ f\u00fcr den Rest des Embryos? Im Moment lautet die Antwort in&nbsp;den meisten F\u00e4llen &#8216;JA&#8217;, diese Zellen sind konkordant (repr\u00e4sentativ) f\u00fcr die anderen.<\/p>\n<p>Die Ergebnisse f\u00fcr jeden Embryo werden durch einfache Definitionen angegeben &#8211; &#8220;zum Transfer empfohlen&#8221;, &#8220;nicht zum Transfer empfohlen&#8221; oder &#8220;nach Unterzeichnung der Einverst\u00e4ndniserkl\u00e4rung empfohlen&#8221;. Aber was bedeutet diese letztgenannte Definition in&nbsp;der Regel? In&nbsp;den meisten F\u00e4llen werden diese Embryonen als mosaikartig oder mit einer leichten Form von Anomalien betrachtet. Mosaizismus beim F\u00f6tus und Neugeborenen ist ein viel selteneres Ereignis als beim Embryo, daher muss es w\u00e4hrend der gesamten Schwangerschaft einen Mechanismus f\u00fcr die Beseitigung abnormaler Zellen geben. Wenn Sie ein PGT-A-Ergebnis erhalten, bei dem einige Embryonen als Mosaik betrachtet werden, ist es besser, einen Termin mit Ihrem Arzt zu planen, um die m\u00f6glichen Risiken und Vorteile des Transfers eines Mosaik-Embryos abzusch\u00e4tzen.
