In several cases (f.e. obstruction of vas deferens) men may have no spermatozoa present in the ejaculate. However the spermatozoa may be retrieved directly from the testes (TESA - testicular sperm aspiration) or epididymis (MESA - microsurgical epididymal sperm aspiration). This procedures are done in our clinic in full anesthesia. Such small surgical operations help to obtain a piece of tissue containing spermatozoa. Even a few spermatozoa are sufficient for the good fertilization result with the help of ICSI procedure.
As soon as fertlization took place, the embryo quality and pregnancy rates after ICSI/TESE/MESE do not differ significantly from those after the usual IVF procedure.
In case of severe non-obstructive oligozoospermia and azoospermia of obstructive and non-obstructive origin, infertility may have genetic causes and can be inherited by the children of male sex or result in genetic disease in offspring. In our clinic such couples undergo medical-genetic counselling and examination (karyotype making, AZF-microdeletions, cistic fibrosis). Only with the help the complex approach we achieve birth of a healthy baby.
